Fill out the application form and make assessment fee payment.

Fill in the Organizational Profile with as much detail as possible and email back for assessment.

Step 3: Employee Commitment Survey

Employee Commitment SurveyIt takes approximately 10 minutes to complete the Employee Commitment Survey. Each participating nonprofit will provide a list of employees taking part in the survey. Each employee will be assigned a pass code to be used to access and complete the survey. All surveys are anonymous and no individual survey data is provided back to the participating organization. On-line surveys are accessible through any desktop or mobile device.
